Community Health Worker


  1. Serves as a liaison/link between public health, health care, behavioral health services, social services and the community to assist individuals and communities in adopting healthy behaviors.
  2. Conducts outreach that promotes and improves individual and community health.
  3. Facilitates access to services, decreases health disparities, and improves the quality and cultural competence of service delivery in Nebraska.
  4. Attend community, department and state meetings, training sessions, seminars, workshops, etc., for professional development, as directed.
  5. Study and identify the different health related issues that exist in a community.
  6. Develop tools, strategies, and resources that can help with those problems.
  7. Help plan and facilitate community meetings, coalitions, and webinars.
  8. Act as an advocate for the community.
  9. Provide referrals for services to community agencies as appropriate.
  10. Work closely with Minority Health Coordinator.
  11. Other duties as assigned.


High school education or GED required.


Willingness to obtain certificate as a Community Health Worker within three months of hire.


  • Ability to work well with individuals of diverse socioeconomic, cultural, and intellectual backgrounds.
  • Excellent advocacy, organization, and communication skills.
