Behavioral Health Patient Services Caseworker
POSITION SUMMARY:
The Behavioral Health Patient Services Caseworker is a member of the integrated health care team. This position acts as a catalyst in the integrated care model, to weave services for patients in the health care and social supports arenas. Caseworker seeks to assist patients with identifying health care needs, behavioral health, medical, dental and eligibility. The employee will screen patients for psychosocial needs and SDoH, using an approved tool. The results of screening will be assessed to determine needed resources. Responsibilities include the development and implementation of a comprehensive referral and resource list to meet those identified needs. Employee will navigate community resources, coordinate areas of care, and monitor utilization of referral placements.
ESSENTIAL JOB FUNCTIONS:
- Responsible for the completion of screening tools, comprehensive assessments, including psychosocial, and Social Determinants of Health screeners.
- Collaborates with other departments and employees to assure all required documents and information is gathered for the patient to include documents that may assist with the patient’s ability to access financial support and all required releases of information to assure continuity of care and HIPAA compliance.
- In collaboration with the patient, assist in identifying programs and services both in the organization and within the community to meet the patients needs to achieve optimal health and wellness.
- Assure referrals to appropriate services within the organization and community are completed timely to improve quality of life.
- Assisting the patient in completing any intake required documents to assure appropriate information is documented in the patient’s chart.
- Assure that patient privacy and confidentiality is maintained at all times, as required by the agency policy and state or federal regulations.
- For behavioral health patients assist with transportation needs and in collaboration with team work to decrease patient no-show rates with patient reminders and confirmations.
- For behavioral health patients, conduct comprehensive initial screening of new behavioral health patients upon entry to behavioral health services and ensure urgent needs identified are addressed in a timely manner.
- Utilize the current E H R to assure that documentation is completed in the chart accurately and information related to referral and follow up is maintained and addressed for patients within appropriate time frames.
- Perform all other duties as assigned.
EDUCATION:
Bachelor's Degree in Human Services, social work, psychology or related field from an accredited school required.
May consider an associate’s degree in a related field with experience comparative to degree requirements.
REQUIRED SKILLS:
- Knowledge or ability to learn Electronic Records System and patient engagement applications.
- Proficient with Microsoft Office Suite or related software.
- Critical thinking skills and ability to analyze complex data sets.
- Ability to communicate effectively and articulate both orally and in writing.
- Ability to work well with individuals of diverse socioeconomic, cultural, and intellectual backgrounds.
- Ability to work within a team as well taking initiative independently.
PREFERRED SKILLS:
- Previous experience working in social services or behavioral health field preferred. .
- 2-3 years of previous experience in related fields.
- Bi-lingual in Spanish preferred.
